Services Offered: Primary Care such as Dental, Vision, OB/GYN, Pediatric Care, Pharmacy, Telehealth, Test; Referral Based Services: Pain Management, Cardiology, Behavioral Health, Diabetes Care, Radiology, Surgery; Emergency Care: Burn Center, Trauma Center, Emergency Department

We offer a sliding fee scale for underinsured and no insurance. Please connect and schedule an appointment with the Eligibility department by calling 602-344-2550. 2. Behavioral health services follow the medical fee schedule and require an internal referral from one our Primary Care Physicians in order to obtain counseling services. 3.  We also assist patients in applying for FES AHCCCS or full benefit AHCCCS. FED AHCCCS (emergency services) cover emergency room fees with diagnosis considered life threatening and or pregnancies. The same documents as self-pay are required to apply for AHCCCS services.
